1. Go to the link below and fill the form of Koya University Staff data collection carefully:

2. Prepare a personal photo with the following specifications:
Photo has to be a passport sized of 50 mm by 50 mm.
The height of the head (top of hair to bottom of chin) should measure 25 mm - 35mm.
Make sure the eye height is between 28 mm – 35 mm from the bottom of the photo.
The photo has to be in white background only!
